• You must report incidents and accidents to the BMG office within 48 hours either by using the on-line report or by e-mail.
• Give as much information as you can, but do not delay submitting the initial report whilst you collect further information – this can be provided later.
• Use this form to report incidents or unintended events as well as accidents.
